Please note that New York Westchester Square Medical Center will no longer participate in the Atlantis Health Plan, Inc. provider network, effective June 19, 2006. Atlantis Health Plan members will continue to have access to hospital services through the 6 facilities in the Bronx that continue to participate in the Atlantis Health Plan provider network.

If your Atlantis patient will require ongoing care at the facility after June 19th, please call Atlantis Member Services at 866-747-8422. The Plan allows the member to continue in ongoing care with the terminating provider for a transitional period of up to 90 days from June 19, 2006. If the member has entered the second trimester of pregnancy by June 19th, the transitional period would include the provision of post-partum care directly related to the delivery and upon the member’s request, delivery at New York Westchester Square Medical Center. The member, their designee, or you should call Atlantis Member Services at 866-747-8422 to request such transitional care. Please note that you as the provider must agree to certain requirements for Atlantis to approve this request.
