PPACA-1 Rider

Patient Protection and Affordable Care Act (PPACA)

• Age 26 Dependents - Coverage for covered dependents up to age 26. Coverage is not restricted on factors such as: financial dependency, residency, student status, employment, eligibility for other coverage, or marital status.
• Pre-existing Condition Limitation - No pre-existing condition limitation on children under the age of 19.
• Preventive Care - Preventive care such as immunizations and preventive screening will be covered with a $0 co-payment.
• Lifetime Limits - The $1,000,000 lifetime max on out-of-network services for essential benefits is eliminated.
• Annual Limits - Any annual dollar limit under your Subscriber Contract that applies to Essential Benefits, whether such annual limit applies only to an Essential Benefit or includes Essential Benefits and other benefits, is hereby deleted.

Vision Low Option Rider

This rider entitles the member to an eye examination once every twelve (12) months, with the benefit period measured from the date of service. Exams are defined by statutory "minimum exam" standards. In addition, the Member is entitled to a $75.00 (retail) eyewear allowance once every twenty-four (24) months, with the benefit period measured from the date of service. Eyewear consists of a frame and/or lens combination or contact lenses.
