FOREWORD
Dear Participating Provider:
Thank you for your participation in Atlantis Health Plan. Atlantis Health Plan is a managed care program licensed by New York State to provide health care. Atlantis Health Plan's objective is to provide affordable quality care to our members through access to the physician and provider network as well as management services to enhance the process.
The Provider Manual has been designed to help you and your office staff understand the Atlantis Health Plan delivery program and to assist in incorporating program activities and procedures (forms, reports, etc.) into your office routine with minimal disruption. Please familiarize yourself and your staff with the information in this manual. Page changes and supplements to the manual will be forwarded from time to time and should be kept with the manual for easy reference.
